
E-141 A, Employee List Form
Required by NJ ABC Law – E-141a form is a list of all of your Employees that will be selling & serving wine. Staff members must be on this list before they can sell or serve wine. Staff must be 18 years or older to sell or serve wine. This form must be kept updated and accessible at all times.

Agreement With Authorized Person
The Agreement with Authorized Person is a required training document for all owners, managers and staff. This form must be completed in full and returned to Old York Cellars before any owner or staff member can begin to serve or sell wine.

Incident Report Form
Wine Incident Report must be filled out if an incident occurs involving Old York Cellars wine and shared with Old York Cellars within 24 hours of the incident. Forms are to be emailed to david@oldyorkcellars.com and anthony@oldyorkcellars.com

Fetal Alcohol Warning Sign
Please post this sign at all times prominently in any service area as well as on a wall, towel dispenser or other appropriate location in any public rest room for patrons on the licensed premises.

NJ ABC Handbook
The NJ ABC Handbook is a legal publication that provides guidelines and regulations for the alcoholic beverage industry in New Jersey.

Alcohol Management Plan
The NJ ABC Act provides that holders of plenary winery licenses and farm winery licenses may operate retail outlets within the State of NJ. See N.J.S.A. 33:1-10(2)(a) and 33:1-10(2)(b). These retail salesrooms are commonly referred to as “winery outlets”.

ID Checking Guide Book
Very Important- Use the copy of the ID Checking Guide Book given to the winery outlet by Old York Cellars to help verify valid ID’s. The book should be kept in a location for all staff to have immediate access to the book.
